When the employee went to flip the mold, the press crushed the employee's right index finger resulting in amputation to the first knuckle.

On September 23, 2017, at approximately 6:40 an employee was helping another employee to remove build up on an energized fabric cutter roller using a putty knife. The putty knife and the employee's hand were pulled into the knives, resulting in amputation of the distal tip of the right middle finger. The machine was not guarded or locked out at the time.
